Risk assessment and clinical and laboratory diagnostics of autoimmune diseases in population affected by the Chernobyl accident
In the last 20 years, there has been an increase in the incidence of autoimmune pathology, including in the population affected by the Chernobyl accident. The State Registry database contains 189 individuals with type 1 diabetes mellitus combined with hypothyroidism that developed as a result of AIT, as well as more than 100 patients with type 1 diabetes and AIT, without realized hypothyroidism. This determines the need for scientific and practical work aimed at identifying early markers for the development of polyendocrine insufficiency in its various combinations (AIT, type 1 diabetes, adrenal cortex and parathyroid gland insufficiency).
Project Activities:
1. To assess the incidence of autoimmune diseases, including autoimmune polyglandular syndromes in the affected population
2. To conduct clinical and laboratory examination and treatment of patients with autoimmune diseases including polyendocrine insufficiency
3. To create a database with the results of clinical and laboratory examination of the population with autoimmune diseases affected by Chernobyl accident
4. To develop methodological recommendations “Algorithm for diagnostics, prognosis and treatment of autoimmune diseases and autoimmune polyglandular syndromes”
Project Activities:
A comprehensive clinical and laboratory examination of at least 200 individuals with realized AIT and/or hypothyroidism will be conducted, including patients with verified AIT and/or hypothyroidism in combination with type 1 diabetes mellitus, including: - assessment of thyroid status with determination of the titer of autoantibodies to thyroid components, determination of thyrotropin levels and free fractions of thyroid hormones; - determination of the titer of autoantibodies to pancreatic β-cells, insulin levels; - determination of the titer of autoantibodies to adrenal cortex cells, determination of cortisol and adrenocorticotropin levels; - determination of autoantibodies specific to other autoimmune diseases.
Based on the data obtained, the severity of autoaggression to target organs and the risk of developing autoimmune polyglandular syndrome in patients with one or more autoimmune diseases will be assessed.
An algorithm will be developed for the diagnosis of polyglandular endocrine syndromes before the clinical manifestation of life-threatening conditions such as diabetic ketoacidosis or acute adrenal insufficiency in young patients.
The project is intended to create a database with the results of clinical and laboratory examination of population with autoimmune diseases affected by the Chernobyl accident for medical organizations in Belarus providing specialized medical care.
Based on a comprehensive examination with the identification of risk factors, it is proposed to develop methodological recommendations “Diagnostics of risk factors for the development and progression of autoimmune diseases of autoimmune polyglandular syndromes”.

Clinical and laboratory examination and treatment of patients with autoimmune diseases, including polyendocrine insufficiency.
